Jia Physio is committed to protecting the privacy of our clients and their families. As a provider of paediatric physiotherapy, we handle sensitive health information in accordance with the Privacy Act 1988 (Cth) and the Health Records Act 2001 (Vic).
2. Collection of Health Information
We collect information necessary to provide clinical care, including medical history, diagnostic reports, and developmental progress. This information is collected directly from parents/guardians or, with explicit consent, from other healthcare providers (e.g., paediatricians or NDIS coordinators).
3. Use and Disclosure
Your child’s information is used solely for the purpose of providing high-quality physiotherapy and clinical advocacy. We do not disclose personal information to third parties unless:
- Written consent is provided by the parent/guardian.
- It is required by law (e.g., mandatory reporting).
- It is necessary to lessen a serious threat to life or health.
4. Data Security & Storage
Health records are stored securely using encrypted, clinical-grade software. We take all reasonable steps to protect your data from misuse, loss, or unauthorised access. In accordance with Victorian law, paediatric records are retained until the child reaches 25 years of age.
5. Access and Correction
You have the right to access your child’s health information held by us. If you believe any information we hold is inaccurate or out of date, please contact us to request a correction.
